unfortunately I have to deal with a poorly performing scenario:

Why can't you use DFS which could be the easiest way to sync files realtime? IIS is only a web hosting feature in windows server which even doesn't have original failover function. So it also depend on the other role/feature to achieve the sycrhonize. The
web deploy could only be used to sync once. For IIS content, it also depend on the DFS role.
For the relation database like sql server, it also have its own way to sync data which could build publisher and subscriber.

Thx for your answer. It apears that DFS-R cannot work work without a NameSpace. Due to bad experience DFS is banned on all AD controllers.
I'm not sure about the DB feature that can sync file-systems, do you have a name for it?

no I do not mean SQL server relication. It might get a little more expensive to deploy "SQL Server" to over 200 Servers.
I need to Sync FileStream content (https://www.red-gate.com/simple-talk/sql/learn-sql-server/an-introduction-to-sql-server-filestream/) in realtime
from the DB via FileStram to the IIS.

I'm sorry that I'm not sure with this feature while it doesn't belong to IIS.
