trying to find out why the site is unable to run wkhtmltopdf.
Right now I don't see any logs under IIS or the event logs.
the application pool has read/execute , list and read permissions.

It is the responsibility of a vendor/tool developers to support running their tools on IIS, not the responsibility of Microsoft to let IIS support all tools.
So in your case, find the developers of wkhtmltopdf and ask them how to make it work on IIS.

So where do you want to invoke the wkhtmltoppdf.exe? asp.net or php application? Based on my understanding, as long as you set the environment variable, you could direct convert html to pdf from command line.
In PHP, as long as you configured the PHP CGI correctly, it should be invoked by:
<?php shell_exec("wkhtmltopdf http://www.test.com/a.html 1.pdf") ?>
Please ensure the application pool identity and authenticated user has enough permission.

got an error, but the exact path or the exact file does not exist. Is it safe to delete all files and folder after the ROOT directory?
why are they there? some of the file dates are old, so a reboot will not remove them.

Its safe to clean the root folder. The folder should be used to place the file that is being dynamic compiled first time. If these file occupy high memory, you could just remove them while they could also cause a low performance with vs.
