    Thanks for working with us directly offline.  It is possible that there might be more than one root cause, but at least one problem seems to be that when IIS Manager is opened from MMC, the ARR icons don't appear.  However, is you open IIS Manager on its own (ie. either start IIS Manager on your client and remote into the server or TS to the server and open IIS Manager), then everything seems to work as expected.

    We will investigate why it doesn't work correctly from MMC.
