Are you running on IIS 7? If so, try changing the application pool to classic mode. I would enable auditing to see if there is some internal error logging being denied. Not having permissions could cause the appliction to be unable and potentially crash
w3wp. I've seen cases where an application logs locally and since either A) it doesn't have permission or B) multiple w3wp are trying to access causing an issue.
Without more errors, we can only speculate.

Then it sounds like you have some domain level policies restricting your auditing rules. Another way around would be to use process monitor to track an individual app pool, identify the Process ID (PID) and monitor with process monitor. Here is an example,
could help if something is being denied.

I am seeing the same issues running Windows Server 2008 and PHP5 installed with ISAPI. So, don't think that will solve your issue. Any other ideas?
