&lt;p&gt;This problem has been dogging me for some months now. So wondered if anyone else had come across it or had a solution to it.&lt;/p&gt;
&lt;p&gt;Set up is as follows:&lt;/p&gt;
&lt;p&gt;Windows Server 2003&lt;br /&gt;IIS 6.0&lt;br /&gt;PHP 5.2.6&lt;/p&gt;
&lt;p&gt;PHP settings:&lt;br /&gt;upload_max_filesize=21m&lt;br /&gt;post_max_size=21m&lt;/p&gt;
&lt;p&gt;FastCgi Settings:&lt;/p&gt;
&lt;p&gt;QueueLength=1000&lt;br /&gt;MaxInstances=10&lt;br /&gt;ActivityTimeout=600&lt;br /&gt;InstanceTimeout=600&lt;br /&gt;InstanceMaxRequests=10000&lt;br /&gt;EnvironmentVars=PHP_FCGI_MAX_REQUESTS:10000&lt;/p&gt;
&lt;p&gt;I have a file upload system on a site for a client hosted as above. The&amp;nbsp;problem I&amp;nbsp;was having before increasing the timeout settings above from 300 to 600. Was any file over around 11mb was causing a FastCGI error (0x800703e3) error number 995 timeout error. Files smaller than this seemed to upload fine.&lt;/p&gt;
&lt;p&gt;Once I increased the settings above everything worked as above except now after about 6-7 minutes I get an http server error 500 on files bigger than 11mb. As you can see the timeouts are set to 10 minutes so this should be ok. The file size variables in php are set to 21m and the largest file I have tried so far is 18.8m so should be no problem with file size.&lt;/p&gt;
&lt;p&gt;The other problem is they are hosting on there own server and I have been liasing with their IT guy to try and resolve this his findings were as follows:&lt;/p&gt;&lt;span style="COLOR:navy;FONT-FAMILY:&amp;#39;Arial&amp;#39;,&amp;#39;sans-serif&amp;#39;;mso-ansi-language:EN-US;"&gt;With the testing I did I proved the following&lt;/span&gt;&lt;span style="COLOR:navy;FONT-FAMILY:&amp;#39;Arial&amp;#39;,&amp;#39;sans-serif&amp;#39;;mso-ansi-language:EN-US;"&gt;&amp;nbsp;&lt;/span&gt;&lt;span style="COLOR:navy;FONT-FAMILY:&amp;#39;Arial&amp;#39;,&amp;#39;sans-serif&amp;#39;;mso-ansi-language:EN-US;"&gt;It was the CGI timings causing large files to time out on the site giving us a error ( files over 10M )&lt;/span&gt;&lt;span style="COLOR:navy;FONT-FAMILY:&amp;#39;Arial&amp;#39;,&amp;#39;sans-serif&amp;#39;;mso-ansi-language:EN-US;"&gt;&amp;nbsp;&lt;/span&gt;&lt;span style="COLOR:navy;FONT-FAMILY:&amp;#39;Arial&amp;#39;,&amp;#39;sans-serif&amp;#39;;mso-ansi-language:EN-US;"&gt;The main problem I was seeing is a big difference between out side traffic and inside traffic&lt;/span&gt;&lt;span style="COLOR:navy;FONT-FAMILY:&amp;#39;Arial&amp;#39;,&amp;#39;sans-serif&amp;#39;;mso-ansi-language:EN-US;"&gt;&amp;nbsp;&lt;/span&gt;&lt;span style="COLOR:navy;FONT-FAMILY:&amp;#39;Arial&amp;#39;,&amp;#39;sans-serif&amp;#39;;mso-ansi-language:EN-US;"&gt;Out side yourself or a customer always timed out 10M or so with an error&lt;/span&gt;&lt;span style="COLOR:navy;FONT-FAMILY:&amp;#39;Arial&amp;#39;,&amp;#39;sans-serif&amp;#39;;mso-ansi-language:EN-US;"&gt;&amp;nbsp;&lt;/span&gt; 
&lt;p class="MsoNormal" style="MARGIN:0cm 0cm 0pt;"&gt;&lt;span style="COLOR:navy;FONT-FAMILY:&amp;#39;Arial&amp;#39;,&amp;#39;sans-serif&amp;#39;;mso-ansi-language:EN-US;"&gt;Inside timed out or froze with files 6M 8M etc usually without an error.&amp;nbsp; So this indicates a timeout with AT&amp;amp;T gateway outwards we use which is very difficult to prove at present. My next step is to try and get simon to route directly to the box over the LAN which is causing concern as the server is sat on a DMZ.&lt;/span&gt;&lt;/p&gt;

There are two IIS Sites running, each with their own application pool and identity. One site is running WordPress and the other is running PHPMyAdmin. When the server first starts or IIS is reset, the site which is hit first runs fine. The other site will get this error:&lt;br /&gt;&lt;br /&gt;&lt;/p&gt;&lt;h1&gt;FastCGI Error&lt;/h1&gt; 
The FastCGI Handler was unable to process the request. 
&lt;hr /&gt; 
&lt;p&gt;Error Details:&lt;/p&gt; 
&lt;ul&gt;&lt;li&gt;The FastCGI process exited unexpectedly&lt;/li&gt;&lt;li&gt;Error Number: -2 (0xfffffffe).&lt;/li&gt;&lt;li&gt;Error Description: Unknown Error&lt;/li&gt;&lt;/ul&gt; 
&lt;h2&gt;HTTP Error 500 - Server Error.&lt;br /&gt;Internet Information Services (IIS)&lt;/h2&gt;&lt;p&gt;--------------------------&lt;/p&gt;&lt;p&gt;&amp;nbsp;I have done the following in my attempt to resolve this issue:&lt;/p&gt;&lt;ul&gt;&lt;li&gt;Reset IIS, and load the site which was throwing the FastCGI error (PHPMyAdmin) before the site that wasn&amp;#39;t (WordPress). The PHPMyAdmin site worked fine but the WordPress site did not.&lt;/li&gt;&lt;li&gt;Reset IIS and did the exact opposite as above (loaded WordPress before PHPMyadmin) and the exact opposite result was achieved (WordPress worked but PHPMyAdmin did not).&lt;/li&gt;&lt;li&gt;Checked permissions for all directories in concern (c:\php; c:\windows\system32\inetsrv; respective website directories). All permissions were fine, as I suspected because the sites do work if they load first.&lt;/li&gt;&lt;li&gt;Ran &lt;b&gt;c:\php\php-cgi.exe d:\websitelocation\phpscript.php&lt;/b&gt; for both sites and the proper result was returned.&lt;/li&gt;&lt;li&gt;Cussed and screamed at the server, PHP, and FastCGI and the expected result was achieved (the vein in my forehead nearly popping). :-)&lt;/li&gt;&lt;li&gt;Checked FastCGI config file (fcgiext.ini) against the documentation here: http://learn.iis.net/page.aspx/248/configuring-fastcgi-extension-for-iis60/.&lt;/li&gt;&lt;li&gt;Configured FastCGI with separate settings for each website. I get&amp;nbsp;an HTTP error 500.0 error.&lt;/font&gt;&lt;/font&gt;&lt;font size="3"&gt;&lt;font face="Times New Roman"&gt;If I run a PHP - info from command prompt it seems to be running but I try the same thing in a script I get the error.&lt;/font&gt;&lt;/font&gt;&lt;font size="3"&gt;&lt;font face="Times New Roman"&gt;IIS unable to access web.config.&amp;nbsp; Solution proposed is to verify the NTFS permission for web.config. &lt;/font&gt;&lt;/font&gt;

I&amp;#39;ve hit the server so hard with requests (using apache bench btw), I&amp;#39;ve done over 100.000 requests to the website, and everything runs just fine. (Although the load is going sky high on this dual xeon 3ghz machine).&lt;/p&gt;&lt;p&gt;&amp;nbsp;Then when I think I finally got everything stable as it should, I get the stupid 500 error again.&lt;/p&gt;&lt;p&gt;The data is:&lt;/p&gt;&lt;blockquote&gt;
