Have you assign appropriate user rights for
PHP folder? If I remember correctly it works for me.

This file is part of my IIS 6. installation, located at windows\system32\setup folder. Are you sure if you were using the right media?

I don't see one, maybe you can convert the config file yourself with code since it is XML.

Yes. I have the same problem.
Html code outputs correctly but <?phpinfo();?> doesn't. I'm sure PHP is running.
